Clinic Information

THE FIRST STEP FOR STUDENTS TO SEE THE NURSE IS TO OBTAIN A PASS AND WEAR YOUR ID CARD! NO EXCEPTIONS.

All health-related information should be given to the School Nurse. She will then disseminate the information to the appropriate persons.

All incoming Freshman and Transfer Students to Rich Central must have a current physical and up-to-date immunization records available to register for classes. 

The School Nurse only accepts the Illinois Certificate of Child Health Examination Physical Form.  All Sports Physicals go to the Athletic Department.

The School Nurse provides professional nursing services, first aid, illness and emergency care to students and staff including nursing assessments, identifying health problems, making referrals for diagnosis and treatment, recommending educational modifications, providing follow-up and evaluation, and maintaining appropriate documentation.  OSHA standards and protocols are maintained. 

The School Nurse participates and communicates as a medical liaison between the home, school, community health agencies and the private medical sector to enhance the health and wellness of the school community.

The School Nurse maintains confidentiality regarding all school and health-related issues.  When requesting records to be mailed or faxed, same day service is not always possible. However, all requests will be expedited within 48 hours.

Medication Policy:

District 227 recognizes that some students may require the administration of medication during school hours. All medications (both prescription and non prescriptions) will be administered by the school nurse. In order for the student to receive medication in school, the District 227 Medication form must be completed by both the parents and the attending physician.
