Rich Central Registration Requirements
Student Information Needed:
-Current Illinois shot/physical
-Birth certificate
-Student Transfer Form (transferring students)
-Withdrawal papers and grades
-Unofficial transcript
-IEP records (if applicable)
-8 th grade diploma
-Guardianship papers (906 forms)/divorce custody papers
-When students are dually-enrolled (Rich Central student attending a full-time program off-campus), parents need to register that student at both the Rich Central and the alternative site locations each year.
Residency Proofs
-Photo ID of parent/guardian
-Mortgage or Lease
-Three (3) current utility bills
